This was the opening fixture in the Round of 16 and a special game for new Genoa coach Davide Nicola, who only made his debut on the bench in the controversial win over Sassuolo, but was here up against his hometown club.
It was Genoa who took a shock lead after 14 minutes, as Francesco Cassata won back the ball in midfield, surged forward, dribbled past a defender and rolled across for Andrea Favilli to turn in relatively undisturbed.
Toro fought back, with Simone Zaza stopped by a last-ditch tackle, but the move continued and Alex Berenguer rolled across for Lorenzo De Silvestri to turn in from 12 yards, although Ionut Radu could certainly have done better.
Genoa remained dangerous, Kevin Agudelo going on a slalom through three defenders to force a one-handed Salvatore Sirigu save, while Soualiho Meite fired straight at Ionut Radu.
Meite was extremely fortunate not to receive a second yellow card for a blatant shirt tug on Lasse Schone on the hour mark.
Sirigu had his gloves stung at the near post by Favilli, who turned Bremer in the box, but Radu had an even more difficult task as he flew to palm an Andrea Belotti header out from under the bar.
Belotti nodded another attempt on to the roof of the net in stoppages after Kevin Bonifazi’s strike was beaten away, as it went to extra time.
Radu was again called into a great save to claw Bremer’s downward header off the line, then Belotti’s free kick.
Meite had risked a second yellow in the first half and he received it eventually, reducing Torino to 10 men for his lunge on Lasse Schone.
Genoa had the ball in the net, but Cristian Romero had wandered offside and it did not count. Favilli and Stefano Sturaro both went for the same Paolo Ghiglione cross, spoiling the move for each other, then Ghiglione went for goal himself and fired just wide.
The two teams could not be separated and it went to a penalty shoot-out that didn’t finish until after midnight local time.
Everyone converted until Sirigu got his gloves to the Ivan Radovanovic effort, so Berenguer finished it off with a Panenka.
